The Clinical Pharmacist works in conjunction with a multi-disciplinary team to provide safe, high quality infusion therapy to patients in the alternate site. He or she is responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the patient while collaborating with physicians, nurses, and other healthcare professionals in an effort to achieve the best possible outcomes.


  • A minimum of two years recent experience in an acute-care setting or one year recent experience in home or ambulatory infusion therapy.  Experience and strong knowledge base in intravenous admixture services and intravenous therapy to include home infusion. 
  • Bachelor of Science in Pharmacy.
  • Doctor of Pharmacy or a Masters of Science degree in Pharmacy (preferred).
  • Currently licensed as a registered pharmacist in state of employment.


  • Adheres to organization policies and procedures.
  • Coordinates and facilitates new referrals/admissions through collaboration with nursing, insurance case managers, and operation.
  • Reviews, receives, and verifies all patient orders and documents according to policy and procedure.
  • Assists the physician and other healthcare professionals involved with planning and implementing appropriate patient care in the ambulatory or home setting to ensure safe and effective drug therapy.
  • Provides timely acceptance and completion of new referrals.
  • Conducts initial and on-going assessment of patient, drug, and disease information to identify potential drug related problems or needs.
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of therapy.
  • Communicates the results of drug monitoring activities to the physician, nursing staff and other appropriate persons.
  • Provides consultation and answers inquiries from patients, family members, physicians, and other healthcare providers regarding medications and their use.
  • Maintains thorough and appropriate documentation in the patient record of all clinical activities and all communication with patient, physician, or other healthcare providers.
  • Attends daily meetings with nursing and other staff to coordinate patient care.
  • Participates in the preparation of progress reports for the referring physician on a regular, periodic basis.
  • Supervises, coordinates, and participates in the preparation of sterile admixtures. Dispenses all pharmaceutical and related supplies/equipment in accordance with federal, state, and local laws.
  • Processes and checks all medications, supplies, and equipment prior to dispensing from the pharmacy.
  • Maintains pharmacy records in accordance with applicable laws and regulations.
  • Identifies opportunities for improvement and communicates with leadership.
  • Demonstrates ability to maintain calm under pressure and cope with changing environment.
  • Participates in on-call rotation as applicable.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature, attending in-services, and other educational opportunities.
  • Participates in the orientation of other staff as requested.
  • Participates in Performance Improvement processes.
  • Performs other duties as assigned.
  • Complies with all federal, state, county, and municipal laws, rules, ordinances and regulations applicable to the nurse practice act in the performance of obligations herein, as well as with applicable PI policies and personnel qualifications.


  • Strong organizational, communication and problem solving skills.
  • A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers.
  • Must be able to assume a flexible work schedule.
  • Must have excellent interpersonal skills in handling interactions with Palmetto Infusion staff, other agencies, groups, patients and families.
  • Competent in use of company email as means of communication, accessing the company website to view company policies and electronic medical records.
  • Competent in the use of company self-service payroll software (time and attendance, benefits, pay statements).
  • Competent in the use of Microsoft Office products and the ability to navigate Internet Explorer.  Proficient in typing, word processing (Microsoft Outlook/Express, Word, Excel), and familiar with office equipment.
  • Proficient in the operation of CPR+.
  • This position requires a knowledge of, and sensitivity to, cultural and language differences.
  • Must have the ability to perform basic mathematical computations.  Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • There are a number of deadlines associated with this position. The employee must also multi-task and interact with a wider variety of people on various and, at times, complicated issues.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job. While performing the duties of this job, the employee may be required to frequently stand, walk, sit, bend, twist, talk and hear. There may be prolonged periods of sitting, keyboarding, reading, as well as driving or riding in transport vehicles. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with patients, vendors and staff.
  • Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team.

The work environment characteristics described here are representative of those an employee encounters while performing the primary functions of this job. Normal office conditions exist, and the noise level in the work environment can vary from low to moderate. Limited overnight travel may be required from time to time.
